Contracted Personal Care

What is Contracted Personal Care?

CLA contracts with Managed Care Organizations and other funders to provide services that assist adults of all ages to live independently in the community.

Services include but are not limited to:

  • Personal cares such as bathing and dressing
  • Light housekeeping and home chores
  • Companionship
  • Community activities

CLA receives a Plan of Care or authorization of services from the organization paying for services. Services are provided in the client home or in the community, depending on client need and specific services that are authorized. To download the Personal Care handout, click here.

Who is eligible?

Individuals are referred through the Managed Care Organization or other funder contracting with CLA for services. To learn more about our contracted services, please contact CLA Intake at (608) 242-8335.

How we help:

  • Recruit and schedule Personal Care Workers that best meet the client’s needs or hire workers of the client’s choice (provided the applicant passes the CLA screening process).
  • Train and supervise Personal Care Workers in the client’s home.
